In the section for "Comments About Type of Appointment", please describe ONLY the type of appointment you desire. Do NOT use the comment section to ask questions or relay medical information to your doctor's office. Messages or questions that are not specific to the type of appointment desired will NOT be forwarded to your doctor.

Clinic Financial Policies

We charge a $25 fee for missed clinic appointments or appointments canceled with less than twenty-four hours notice. We charge $50 for missed procedure (urodynamics, cystoscopy, etc) appointments or appointments canceled with less than twenty-four hours notice. These charges are not billable to your insurance company and you will be responsible for payment of this charge. Missed appointments often mean that someone else was not able to be seen in a more timely fashion. Please be courteous, cancel or reschedule your appointment as early as possible.
