Please arrive 30 minutes prior to your appointment to check-in and ensure your paperwork is complete.

NOTE: If this is your first appointment, please be sure you have completed EVERY PAGE of the intake questionnaire for YOUR doctor. Without this important information, YOUR APPOINTMENT MAY BE DELAYED OR RESCHEDULED.

When you come for your appointments, please remember to bring the following

• Driver's license or State ID
• Insurance Card/ Information
• Copies of surgical/ procedure records, medical records, x-ray reports, MRIs, CT scans and so on from prior doctor visits
• Medications list (if any)

To cancel an Appointment

Telephone the office during business hours. Please cancel at least 1 day ahead so that your appointment time can be allocated to another patient who is seeking treatment.
