Please arrive 30 minutes prior to your appointment to check-in and ensure your paperwork is complete.

When you come for your appointments, please remember to bring the following

• Driver's license or State ID
• Insurance Card/ Information
• Copies of surgical/ procedure records, medical records, x-ray reports, MRIs, CT scans and so on from prior doctor visits
• Medications list (if any)

In addition, if this is your first appointment with us, please make sure that you have completed EVERY PAGE of your intake questionnaire for your doctor. IF YOU HAVE NOT, YOUR APPOINTMENT MAY BE DELAYED OR RESCHEDULED.
